2. Complete the Cognitive Assessment form (download the form below).
3. Complete the lab work ordered by our physicians. These will be ordered after your initial visit.
4. Once we receive your lab results, we will call you to schedule your follow-up visit to go over your lab results, and initiate your treatment plan.
*Also included in the cost of the program are: 2 consults with a nutritionist, 1 session with our Life Coach, and unlimited phone consults with our medical assistant when questions arise.
