We are in-network with some United Healthcare plans for Functional Medicine Services.
For out-of-network insurance carriers, we can provide you with a Superbill to submit to your insurance company upon request. Some insurance plans, especially PPO's, will provide you with reimbursement for your appointment payment. Many lab fees are billable to your insurance plan, including plans that we are not in-network with.
**While we are in-network with United Healthcare, please check with your insurance carrier as we are not in-network for certain plans within that networks. We recommend that you contact your insurance company to know the coverage details of your appointment and lab coverage prior to your appointment. We will then be in a better position to discuss and choose lab options that may be billable to your insurance.
Note: All Physical Therapy and Acupuncture Services are out of network.
Functional Medicine: Self-Pay
Initial New Patient Appointments are $315.00. For extended initial appointments, additional charges will be incurred.
Follow-Up Appointments are typically 45-60 minutes, billed at a rate of $250 per hour based on the appointment time.
If an appointment lasts over the scheduled appointment time, we prorate those fees accordingly.
$300 per hour for ENabling COgnitive REpair (ENCORE program) appointments Appointments are 90 minutes.
*Appointment costs do not include lab or treatment costs. Many lab fees are billable through your insurance. Please contact your insurance company to know the details of your lab coverage prior to your initial appointment so we know what your most affordable lab options are.
Functional Medicine: With Insurance
Our Functional Medicine Providers are in-network with some United Healthcare Plans.
Your co-pay is collected at the time of service. We will bill United Healthcare for you.
Some PPO plans do provide reimbursement for out-of-network services. We are happy to provide you with a superbill if you have a PPO insurance plan.
We are also able to collect Health Savings Account (HSA) and Flexible Savings Account (FSA) payments.
