What Is Pelvic Floor Dysfunction?

Pelvic floor dysfunction (PFD) encompasses numerous conditions affecting the reproductive, sexual, gastrointestinal, neuromuscular, urinary, and other systems in and around the pelvis. PFD can include issues related to organs, connective tissue, muscles, joints, and nerves. 

 

Throughout a woman's lifespan, many things can go wrong regarding the pelvic floor muscles and nerves feeding them. As a result, muscles may be too weak, too tight, full of scar tissue, or in spasm–all issues that make it difficult for them to do their jobs properly.

Urinary Incontinence

Pelvic Pain

Pelvic pain can affect your quality of life and ability to thrive. Pelvic functions include supporting organs, controlling bowel, bladder, and sexual function, breathing, pregnancy, and childbirth. Pain can manifest as trigger points, overactivity, or scar tissue. Working with a knowledgeable Pelvic PT, trained to use FSM, is key to unlocking the drivers of pelvic pain.

Interstitial Cystitis

AKA painful bladder syndrome, IC is a chronic pain condition that causes pain, pressure, and discomfort in the bladder and pelvis without another diagnosis to explain the pain. With FSM, Pelvic PT, behavioral modifications, and exercises, it is entirely possible to experience significant decreases in pelvic pain and urinary symptoms. We are here to help! 

Pelvic Organ Prolapse

Feelings of something falling out, heaviness, or pressure? Think of the pelvic floor as a hammock. One of the most important jobs is to hold the pelvic organs in place. POP occurs when there is weakness of the vaginal wall that causes pelvic organ(s) to press into the wall of the vaginal canal.

Endometriosis

Do you experience excessive cramping pain during your period along with heavy or abnormal menstrual flow? This could be due to endometriosis. It is a painful condition in which cells that usually line the uterus (endometrium) grow outside of the uterus on other tissues and pelvic or abdominal organs. Pelvic PT and FSM can help!

Integrative PT for the Pelvic Floor

At The Healing Collective, a primary goal of our Integrative Physical Therapy to achieve pelvic health involves downregulating the nervous system to decrease or eliminate pelvic pain. Additionally, Dr. Madelyn helps clients who are experiencing incontinence to go for a jog or sneeze without leaking urine, while other clients can once again engage in sexual intercourse without pain. 

 

Because pelvic pain is limiting in any capacity, many clients have experienced an unwelcome disruption to their daily lives. Dr. Madelyn uses all of this information, as well as the client's input and goals, to tailor a plan of care to address unique needs.

There is Still Hope for “Nonresponsive” Pelvic Pain!

If you have tried Pelvic Therapy before and haven’t achieved the results you were seeking, you are unfortunately not alone. Recent studies demonstrate that while pelvic floor muscle training can be effective for many conditions, there are subgroups of non-responders to conventional pelvic therapy. These individuals have tried to seek relief from their pelvic conditions but have fallen through the cracks of a standard therapeutic approach. And not everybody needs to do kegels! Some pelvic conditions involve hypertonic muscles, meaning the muscles are too tense and cause pain, and kegel exercise can actually make these symptoms worse. Other conditions require neural regulation in order to calm the system and reduce tension and pain.


Pressure regulation is often overlooked or misunderstood and is key to an optimal system. Dr. Madelyn has found that many “nonresponsive” pelvic conditions actually result from the presence of adhesions that can be from an old or even forgotten injury that prevent the optimal flow of blood or nerve activity. People with adhesive tissues are often dismissed when they do not respond to standard techniques, but with the support of the leading edge therapy offered through FSM, we can often make a significant change in as little as one session when this is the root cause.
